RISE in major European project to support people with chronic diseases

RISE Research Institutes of Sweden is the coordinator of the new European research project CarePath, which aims to improve the lives of people with chronic diseases. By developing innovative, personalised tools, the project will strengthen the individual's ability to follow their treatment – and thus contribute to better health, increased quality of life and reduced healthcare costs.
"CarePath is an important step towards more integrated and individual-centred care. By combining technology, research and user insights, we want to create solutions that really make a difference in people's everyday lives," says Paul Halle Zahl Pedersen, Senior Vice President, Digital Systems Division at RISE.
Despite medical advances, many people find it difficult to continue their treatment over time – often due to a lack of support, social barriers, or financial pressure. CarePath addresses these challenges by developing a flexible toolbox that can be adapted to different healthcare settings and the needs of individuals.
The project focuses on three common chronic conditions – obesity, type 2 diabetes and cardiovascular disease – and includes both primary care (e.g. health centres and pharmacies) and specialist care (e.g. hospitals and clinics).
"We want to create a new ecosystem for the care of chronic diseases, where every patient receives the right support at the right time. It is about improving adherence to treatment and thus also health outcomes, but also about reducing healthcare costs," says Jessica Dagman, Senior Project Manager at RISE.
The CarePath toolkit will include:
- Mobile apps that give individuals the opportunity to take an active role in their care and contact with healthcare
- Tools for measuring patients' experiences and health status (PROMs and PREMs)
- Simple and easy-to-use questionnaires that identify the need for extra support, for example in case of pain, fatigue or lack of information
- Combinations of digital solutions and human support that can be adapted to different healthcare environments
To ensure that the solutions are relevant and usable, the project will involve patients, healthcare professionals and other stakeholders throughout the development process. It will also review existing strategies to understand what works best for different people in different situations. CarePath will pilot in six European countries, including Sweden, based on a variety of healthcare systems and communities.
"CarePath also creates new opportunities for health innovations that integrate tools for compliance, which can open up for market development in digital health," says Camilla Evensson, Project Manager for Digital Health at RISE.
