Sweden's healthcare system is facing major challenges. Healthcare costs are rising sharply, the number of hospital beds is decreasing, staff are overworked, and the population is ageing. Chronic diseases account for 80 per cent of healthcare costs, yet the systems are designed for emergency care rather than preventive healthcare. Many digital solutions are tested but never implemented in practice, and different systems are unable to communicate with each other.

RISE supports municipalities, regions, and businesses in transitioning to future healthcare and social care. We carry out needs analyses, lead implementation projects, and develop AI solutions for areas ranging from diagnostics to mental health. We certify digital safety alarms and person-centred care, ensure privacy protection for health data, and establish innovation partnerships that bring together municipalities, companies, and researchers. With our broad interdisciplinary expertise and long-term understanding of systems, we can help you to navigate technical and organisational changes.

I Sverige uppskattas det leva omkring 68 000 kvinnor och flickor som utsatts för könsstympning. Med en åldrande befolkning kommer fler och fler äldre att ha en bakgrund där de har utsatts för könsstympning. Trots det finns det enligt Socialstyrelsen i princip ingen forskning och mycket liten klinisk erfarenhet kring äldre kvinnor och könsstympning.

Det saknas kunskap och forskning om vilket stöd äldre kvinnor som utsatts för könsstympning behöver. Även informations- och utbildningsmaterial för yrkesverksamma inom äldreomsorgen lyser med sin frånvaro. Nationella aktörer som SKR och Socialstyrelsen erbjuder stöd riktat till personal som möter barn, unga och kvinnor i fertil ålder – men inte till dem som möter äldre kvinnor. 

Därför behövs en behovskartläggning som beskriver behoven inom den kommunala omsorgen och behoven hos äldre kvinnor som utsatts för könsstympning. Utan en sådan kartläggning är risken stor att de lösningar som tas fram träffar fel. Att en behovskartläggning saknas riskerar att orsaka dels mer lidande för kvinnorna, dels ökad frustration och känsla av otillräcklighet hos omsorgspersonalen.

I projektet Rätt till hälsa genomfördes därför en sådan kartläggning där intervjuer med kvinnor i målgruppen, samt med personal från den kommunala omsorgen användes för att identifiera och definiera kunskapsgap och behov. 

I projektet ansvarade RISE för kvalitet i utformning och genomförande, exempelvis vid intervjuer, analys och slutrapportering. 

Projektet koordierades av Jämställd Utveckling Skåne, som arbetar med strukturellt förändringsarbete kopplat till de jämställdhetspolitiska målen, tar fram analyser, gör kartläggningar, utbildar och processleder, utvecklar och implementerar metoder samt utvärderar och följer upp jämställdhetsarbete.

I projektet deltog även Existera, som arbetar mot könsstympning av flickor, ger stöd till kvinnor som har blivit utsatta för könsstympning, samt arbetar för att motverka att fler flickor utsätts. Existera utbildar och ger råd till yrkesverksamma, till exempel personal inom socialtjänsten och skola.

CarePath - Research for patient engagement and treatment adherence

The CarePath project aims to improve the management of chronic diseases such as obesity, type 2 diabetes and cardiovascular diseases. Through its smart, AI-powered toolkit of learning algorithms, it designs individualized support in real time, increasing the likelihood of adherence for better health and lower healthcare costs.

RISE coordinates CarePath, a large-scale EU project designed to revolutionize the management of chronic diseases such as obesity, type 2 diabetes and cardiovascular diseases. The overall goal of the project is to radically improve patients' adherence and long-term persistence with their medications. This in turn will lead to significant improvements in the health and quality of life of individuals, while also contributing to significant cost savings and reduced burden on healthcare and health systems.

CarePath operates in 13 different European countries, and pilot studies in 6 countries, Sweden, Germany, Israel, the Netherlands, Poland and Spain. This wide geographical spread ensures that the solutions are relevant and adaptable to different care settings and socio-economic conditions.

At the heart of the project is a smart, AI-powered toolkit with learning algorithms. This innovative toolkit integrates existing digital and analogue health solutions and tailored modules, enabling real-time personalized support. The toolkit facilitates communication and data exchange between patients, health professionals and other stakeholders, strengthening the entire care chain. Particular emphasis is placed on making information available in an understandable and actionable way to counteract the effects of low health literacy.

Prior to the development of the toolkit, a comprehensive needs analysis and pre-study is carried out to map existing successful and less successful intervention strategies and identify barriers and opportunities. This is followed by intensive design work in close collaboration with ecosystem partners, including patients and health professionals, to tailor and optimize effective solutions.

The effectiveness of the toolkit will be tested and validated through four rigorous studies: three pilot studies conducted in 6 different countries of which Sahlgrenska, Sweden is a representative and finally a controlled Proof-of-Concept study conducted in Germany. These studies include patients in both primary and specialized care, with a particular focus on individuals at high risk of low adherence to treatment. They include patients using weight loss medications (such as dual GIP/GLP-1 receptor agonists), lipid-lowering drugs, insulin and SGLT2 inhibitors. CarePath thus aims to create a more sustainable, patient-centered and efficient future for the management of chronic diseases in Europe.

Advancing heart failure management through AI-driven telemonitoring

In AIcares, we are combining AI-driven telemonitoring with specialised home care environments to improve the management of heart failure patients. Telemonitoring, which enables the wireless transmission of patient health data for remote management, is becoming increasingly common around the world. This proactive approach reduces the need for hospitalisation, in line with the digital transformation of global healthcare. Patients with advanced heart failure often require recurrent hospitalisation, posing significant challenges for both patients and healthcare systems.

We aim to develop a predictive AI-driven model to identify heart failure patients at high risk of mortality within one year by analysing electronic health records. We will also implement an intuitive interface using language models to communicate predictive results to healthcare professionals to facilitate their decision-making.

Our goal is to provide cost-effective, personalised and safe care for patients with advanced heart failure. By combining telemonitoring, specialised home healthcare and AI-based predictive modelling, AIcares has great potential to both improve patient outcomes and reduce healthcare costs.
